About Dobutamine Adrenergic Agonist Agent Cardiac stimulant in CHF Directly acting inotropic agent. Mechanism of Action of Dobutamine Dobutamine exerts its pharmacological action by selectively stimulating beta-1 adrenergic receptors. It is a positive inotropic drug and increases stroke volume and cardiac out put without significant change in heart rate peripheral resistance and BP. It improves coronary blood flow and myocardial oxygen consumption. Indications for Dobutamine 1. Acute myocardial infarction 2. Acute heart failure 3. Cardiogenic Shock 4. Septic Shock 5. During cardiac surgery 6. Chronic CHF Interactions for Dobutamine Bretylium: Potentiates its action resulting in arrhythmias. Halogenated hydrocarbon anaesthetics: Risk of serious arrhythmias. Oxytoxic drugs: May cause persistent hypertension. TCAs: Use with caution since efficacy of vasopressor enhanced. Phentolamine & Prazosin: Antagonistic action. Nitroprusside: Additive effect.
